MISSOURI TEAMS DO NOT NEED A PERMISSION TO TRAVEL FORM
Online Check-In Documents
To register, all players/coaches/teams must have the following uploaded through online check-in:
1. Current player and coach passes
2. Current state approved roster
3. Guest players must have the same information to register, along with guest player permission form
4. Non-Missouri teams must have an approved “permission to travel” from their state affiliate
TEAM DOCUMENTS AT GAMES
It is the responsibility of each team to have the following in their possession at the start of each tournament game, able to present to the referee if requested:
- Signed and dated USYS Medical Release Form for each player
- Current player & coach passes for all players/coaches including guest players
- Guest player permission form if applicable
- Current state approved roster
